DiseaseTrak understands the importance of the clinician/patient relationship and the ability for clinicians to closely monitor their patients.

This portal allows clinicians to access information in real-time, to input data about their patients enrolled in a DiseaseTrak managed program, and to view their patient’s progress throughout the program. Information made available to clinicians can include patient status, disease progression, questionnaire answers, therapy management, financial balances, and medication utilization and compliance history.

Clinicians can login to the portal anytime/anywhere with an internet connection, a secure login and password. The ease and flexibility of the portal are key features that keep our clinicians engaged and returning.

DiseaseTrak can also manage, for clients, various programs in which the clinician is a participating member where a clinician is able to track lab data and/or answer defined questionnaires relating to studies or patient follow up.
