DiseaseTrak is a patent-pending software platform used to collect patient healthcare data for partners and subscribers. Through our web-based portals (our state-of-the-art customized interfaces), our software can respond to requests and aggregate data from patients, pharmacies, clinicians, service providers, and pharmaceutical companies. Our portals are easily accessible anywhere, anytime, via the internet, using our secure web server.

The functionality and security level of each portal can be customized to the partner’s needs and for different user groups. Upon an authenticated login and password, and based on the user’s defined level and authority to access information, members are directed accordingly to their home page to begin their search. Role-based security is provided for all users, so that the user only sees the data and information intended for that user.
