Education and Training
When a patient walks out of their healthcare provider’s office everything they have been told walks out with them. At DiseaseTrak, we understand that consistent and constant communication is essential to eliminate the stress and confusion that patient’s experience. With the coordination of their healthcare providers, our expert agents provide
patients with information designed to enhance a patient’s understanding of their disease and their treatment.
Our communication begins with our first patient outreach and continues throughout our care. Several methods of
delivery are offered to every patient to invoke involvement and uptake.
   
Compliance and Persistence
 
Compliance and persistence are key components for maximizing the effectiveness of a therapy and improving patient outcomes. DiseaseTrak assists in designing and managing C&P programs for clients that are best suited for the patient and therapy. In addition to other program features, we can track dispensing activity to the prescriber level and provide data identifying compliant and non compliant patients. We believe that behind the success of every program there is a program that was built with the feedback of patients and healthcare providers.
