DiseaseTrak’s patient management services provide patients with the freedom that they deserve and with the confidence of knowing that we are unified in our commitment to them. Patients are often overwhelmed and need the ability to have choices while making adjustments to their new regimen. At DiseaseTrak, our patients are provided with options that empower them to take control of their new lives while knowing that they are never alone and that we are only a phone call or click away.

Therapy Management
As patients display confidence in their level of understanding of their
disease state and treatment they often forget the essential things that
will assist them in achieving the best outcomes. With that in mind,
DiseaseTrak monitors each patient’s treatment regimen, dispensing history, medication usage and response to
therapy (safety and effectiveness) and provides patients with reminder tools and an interactive calendar to help them
manage their care.

Disease Progression Monitoring
DiseaseTrak understands that patients will reach peaks and valleys in their disease progression and that we need to be there to support them. We measure the changes to a patient’s conditions and symptoms and provide feedback to patients and their healthcare providers. Visual representations (charts and graphs) are supplied so that patients can physically see their disease progression and have informed discussions with their healthcare providers.
