Patient Portal

 
The Patient Portal (MyDiseaseTrak.com) was created to provide patients
support in a variety of programs. Patients are assigned a system generated de-identified number, a program number, a login and password and are only able to view their own information within the portal. The portal allows patients to access tools and services to assist them in managing and monitoring their treatment and disease. Therapy management, disease progression monitoring, compliance and persistence tools, and patient education and information are some of the tools and services we offer in the Patient Portal.
 
As the program requirements dictate, patients can also be asked and
reminded to complete questionnaires based on their therapy and/or disease state. Questionnaires can be provided by MedCom teams, clinical trial departments, or the clinicians involved in the patient’s care.
 
The portal can also be used for reference materials, satisfaction surveys, or targeted marketing, and can provide links to further information about a patient’s disease, web-based research, or other products. Our clients also have the ability to customize the Patient Portal to truly provide “one place” for all of their patients’ needs.
